Overview

All requests for academic accommodations must be based on documented need. Students are responsible for providing appropriate documentation to the Accessibility Centre and Education Support (ACES). ACES works collaboratively with students, faculty, and campus partners to develop individualized accommodation plans and support equitable access to learning.

What is Credible Documentation?

Students must submit supporting documentation from a qualified healthcare or mental health professional that confirms a diagnosis or condition, outlines functional limitations, and explains the impact on academic functioning. Documentation should be relevant to a post-secondary environment and sufficient to support accommodation planning.

Students and providers may use the ACES Medical Documentation Verification Form as a means of meeting ACES documentation requirements.

Important Considerations

Documentation must include recommended accommodations; however, we cannot guarantee that all recommended accommodations will be accepted. Additional or updated documentation may be required.

Mental Health and Counselling Supports

We encourage students to access Counselling and Psychological Services (CPS) for confidential mental health support and documentation review. Visit CPS for more information on their services.

Accommodation Process

  1. Register with ACES and submit documentation
  2. Attend an intake appointment
  3. Develop an accommodation plan
  4. Share accommodations with instructors
  5. Access ongoing supports

Documentation Requirements by Disability

The following drop-down menu guide provides a list of medical conditions and the supporting documentation necessary for requesting accommodations due to the respective condition. This is meant to help students and providers understand what information ACES requires.

Assessment/report from a qualified healthcare professional confirming diagnosis and outlining functional limitations affecting postsecondary studies.

Assessment by a qualified professional (e.g., psychologist, psychiatrist, pediatrician) identifying impairments and academic impact, as per the CADDRA Guidelines. You may also use the ACES Medical Documentation Verification Form. Prescription alone does not confirm diagnosis.

Assessment from a registered psychologist or psychiatrist confirming diagnosis and outlining functional limitations. High school records (IEP, transition plan) may also be helpful.

Assessment from appropriate healthcare professional outlining diagnosis and functional limitations. You may also provide supporting educational records.

Assessment by an audiologist or qualified professional describing hearing impairment and academic impact. You may also provide supporting educational records.

ACES requires a complete psychoeducational assessment. The assessment must

  • either including testing that uses adult measures (i.e., WAIS) or, if the assessment uses childhood measures (i.e. WISC), the documentation cannot be more than 5 years old.
  • The assessment should come from an Educational Psychologist, as per federal guidelines. If a school Guidance Counsellor completes your psychoeducational assessment, we will accept the documentation and implement temporary accommodations to give the student time to obtain an assessment from either an Educational Psychologist or Registered Psychologist.
    • The CSG-DSE grant through Student Aid is available to help cover the cost of a new psychoeducational assessment
    • Some family healthcare care plans will cover the cost or partial costs of this assessment.
