Release of Information for Prospective Students
Next of Kin release
When completing your application to Memorial, you will be asked if you want to authorize the University to release information regarding your application and admission to your identified next of kin.
If you select "Yes", you authorize the Registrar’s Office to release information on your application, to the identified next of kin on your application, concerning your application, supporting documents, admission status, entrance scholarship (if applicable), and letter/verification of enrolment for your first semester at the University following admission.
This authorization will end after the deadline to drop courses without academic prejudice in the semester to which you applied. You may end this authorization at any time by emailing the Registrar's Office at email@example.com.
Admissions representative (agent) release
If you are using an agent to assist with your Memorial application, you can release information about your application to them. To do this complete the Admissions Representative Waiver form.
By submitting the completed Admissions Representative Waiver form you authorize the Registrar’s Office to release information on your application, admission status, and transfer credit status to the identified agent.
To end this authorization you must make a separate email request to the Registrar's Office at firstname.lastname@example.org.
