Vaccination declaration deadline Sept. 7

As a part of our shared responsibility to keep each other and our extended families safe during the upcoming school year, COVID-19 vaccines are required for all students, faculty and staff. As a reminder, first doses of the vaccine are required by Sept. 7 and second doses are required by Oct. 15, 2021. Memorial has launched the secure vaccine declaration form. Please take a few moments to complete this form at your earliest convenience and no later than the end-of-day Sept. 7. You can access this form directly in the My MUN portal.

This form integrates the process for exemptions to the vaccine mandate. If you submit by Sept. 6, you’ll be prompted to enter a prize draw for one of 20 prizes including parking permits, gift cards and items from Memorial’s bookstore.

Reminders:

• Please do not send documentation directly to your department. Memorial values the privacy of your information and the form is the most secure way to declare your vaccine status.

• Residents of Newfoundland and Labrador who have received the COVID-19 vaccine can access their vaccination record online.

• To get vaccinated, please find on-campus vaccine clinic information or regional health clinic information on Memorial's website.
