Report an Incident (Hazard, Near Miss or Accident)
Any incident that involves injury to a person or damage to property, or had the potential to do so, must be reported to the Environmental Health and Safety within 24 hours of occurrence.

An Accident Incident Report Form must be completed for every incident, even if there were no injuries sustained. If the incident involves a student or visitor to campus, the report form must still be completed.

Serious Incidents or Accidents
Serious accidents or incidents must be immediately reported to Campus Enforcement and Patrol
- Death
- Collapse or structural failure of a building, tower, crane hoist, temporary construction support system. or excavation
- Explosion, fire, or flooding
- Fracture of a major bone
- Amputation, or cuts where major blood volume is lost requiring hospitalization or time off work
- Loss of sight
- Third degree burns
- Unconsciousness resulting from concussion, electrical contact, or asphyxiation
- Poisoning
- Any injury resulting in paralysis
- Any other injury likely to or had the potential to endanger life or cause permanent disability.
Near Miss
Near Miss are incidents that do not result in injury but had the potential to.
1. Report the incident to your supervisor immediately.
2. The supervisor must report the incident to the Environmental Health and Safety within 24 hours of the incident.
3. The supervisor and employee review the recommendations of corrective action with a focus on prevention of recurrence.
