Accident/Incident Reporting

If an employee suffers an injury while at work:

  • Seek medical attention or call emergency contact numbers applicable for your area.
  • Report your injury to your supervisor immediately.
  • Assist in completion of the accident incident report when possible.
  • Cooperate during the investigation.
  • If you suffer an injury while at work and are an employee that is covered by Workers Compensation, complete and submit the WorkplaceNL Form 6 - Worker's Report of Injury.
  • To submit a Form 6 (Worker’s Report of Injury) to WorkplaceNL:
    • use the secure MyWorkplaceNL online portal for the fastest service
    • alternatively, fill out the paper form and mail to: P.O. Box 9000, St. John's, NL A1A 3B8
