VIP Forms

The forms below are for the purpose of the Volunteer Incentive Program.

The Organization form will be used to verify your volunteer hours.
The Professional/Personal Development form will be used to verify your professional development hour(s).
The Leadership Development form is for GOLD participants ONLY and will be used to verify their leadership development activity.

VIP Forms for the Winter Semester are due on April 7th, 2026 by 4pm (email to vip@mun.ca)

Remember, forms are due back to the SVB on the last day of classes in the semester (April 7th, 2026, by 4pm) in which you are participating in the VIP! You can submit these forms by emailing them to us at vip@mun.ca. Please ensure that when you email us your forms, you email them all together and not separately. We no longer accept physical forms at the office.
