Additional forms

In addition to the online application and general required documents (e.g., transcripts, confirmation of degree completion, resume), some academic units require that additional forms or documents (see updated list below). If this applies to the program you are interested in, please upload the required form or document to the online application. In the case of writing samples, this can be a term paper, essay, article, thesis, or similar piece of work.

  • Anthropology: Sample of writing

  • Archaeology: Sample of writing

  • Arts and Education (Master of Arts and Education): Sample of writing in French

  • Business Administration: GMAT

  • Education (PhD): Sample of writing

  • English: Sample of writing

  • Environmental Policy: One page statement outlining research interests

  • Ethnomusicology: Sample of writing

  • Fine Arts: A portfolio of 20 images of examples of work or other documentation of visual art research (and a sample of academic writing maybe required)

  • Folklore (PhD only): Sample of writing

  • Gender Studies: Sample of writing

  • Health Ethics: Sample of writing

  • History: Sample of writing

  • Humanities: Sample of writing

  • Interdisciplinary (PhD): detailed research proposal and 3 Letter of Commitment forms

  • Management: Sample of writing

  • Political Science: Sample of writing

  • Psychology: GRE General Test (for all programs except behavioural neuroscience)

  • Psychology (PsyD): 3rd reference letter (including contact information of a person who can speak to personal suitability for clinical psychology), personal statement (view the PsyD website for more information)

  • Psychology (Master of Applied Psychological Science): GRE General Test and sample of writing

  • Social Work (Master of Social Work): Supplementary Information Form

  • Social Work (PhD): Supplementary Information Form

  • Sociology: Sample of writing

  • Transdisciplinary Sustainability: The "Statement of Interest" section of the online application must demonstrate the applicant's understanding of and interest in the transdisciplinary approach (i.e. co-production with partner organizations, integration of distant disciplines). The potential supervisor should also email the Graduate Officer confirming the amount of top-up funding they will provide to support the student each year.
