Students, faculty and staff are required to have a first dose of an approved COVID-19 vaccination administered by Sept. 7, 2021, and the second dose by Oct. 15, 2021, unless granted an exemption. Recognized vaccines include those approved by the World Health Organization (WHO) and/or Health Canada.

Memorial has a secure vaccine declaration form. You can access this form directly in the My MUN portal. If you have any technical issues accessing the form, please contact the help desk. As of Sept. 22, Memorial has received forms from approximately 99 per cent of employees and 83 per cent of students. Of these respondents 95 per cent of employees and 86 per cent of students have received two vaccinations.
This form was due on Sept. 7, and if you have not done so, please complete it at your earliest convenience. This form integrates the process for exemptions to the vaccine mandate.
Residents of Newfoundland and Labrador who have received the COVID-19 vaccine can access their vaccination record online. To get vaccinated, please find on-campus vaccine clinic information or regional health clinic information on our website.
A new procedure under Memorial’s Health and Safety policy is also available to help support implementation of this mandate.
