Biosafety Certificate Application

A MUN biosafety certificate is required for all activities (research and teaching) that involve the use or manipulation of biohazardous agents, and materials containing such agents which are:

  • Conducted on University premises, or in a building or location administered by or under the control of the University,

  • Undertaken by University personnel (staff or students), or

  • Funded through University-administered funding.

A valid biosafety certificate is required before the University will release/continue funds for any grant that involves biohazards.

New Applicants/Renewals

The following forms are available to researchers who need to obtain a new biosafety certificate or renew an existing certificate. All forms are fillable and instruction for completion are provided within each document.

New applicants: Completed application forms, including the Biohazard Procedures Risk Assessment and Matrix for Assessment of Risk Group (the matrix is required only if the RG is unknown) must be forwarded to the BSO AT LEAST SIXTY (60) DAYS PRIOR TO THE DATE APPROVAL IS REQUESTED.

Renewals:  Complete renewal applications must be received by the  BSO NO LATER THAN NINTY (90) DAYS  PRIOR TO THE CURRENT CERTIFICATE’S EXPIRATION DATE.

Use of certified electronic signatures is highly recommended.

Please forward completed application packages via email to bso@mun.ca (Simply save the pdf with electronic signatures, and email the file to the BSO. Paper applications will not be accepted).

Only current versions of the application form will be accepted (current version is 3.11) (older versions will be returned to the applicant for re-submission).

Current Biosafety Certificate Holders - Amendments

For current biosafety certificate holders who intend to modify or change aspects of their research with biohazards (eg. change in procedures, biohazards, locations, etc.), an amendment to their biosafety certificate must be approved by the IBC.

Please forward completed application packages [including the biosafety certificate amendment request form, biohazard procedures risk assessment (if applicable) and matricies for the assessment of risk group (if applicable)] via email to bso@mun.ca (paper applications will not be accepted).

Only current versions of the amendment form will be accepted (current version is 3.4) (older versions will be returned to the applicant for re-submission).
