This report documents the event that you attended/organized and highlights key developments. This form can be completed online, downloaded as a PDF file, or picked up at Student Affairs and Services UC-5029.

If you have photos that you would like to share, you can email them to us at innovate@mun.ca or if you only have a hard copy they can be droped off or mailed to our office where we can scan them and return to you shortly after. If you have any questions regarding the purpose of this documentation or photos, please do not hesitate to contact us.
