The most efficient way to request an official copy of your
academic transcript is by using Memorial Self Service.
Log in using your student number and PIN*, then click on the link
to the Academic Information Menu, and follow the instructions
labelled 'Request Official Transcript'. Once you have submitted
your request, you may check its status on the same web

address below a written transcript request stating:
For your security, we cannot accept requests for official
transcripts via e-mail. Only requests bearing your signature,
either in writing or electronically through the secure access to
Memorial Self Service, can be processed.
All students, both current and former, have access to Memorial
Self Service. If you have used Memorial Self Service or the
telephone registration system before, your PIN will remain as you
specified. If you have never used the Self Service or telephone
system, or if you have recently applied for re-admission to
Memorial, you will have an initial six-digit PIN comprising your
birth year followed by your two-digit day of birth. Feel free to
contact us if you have difficulty accessing your account.
The Office of the Registrar is pleased to provide regular mail service for official transcripts free of charge.
The award of an academic Program Credential (Degree, Diploma
and/or Certificate) is reflected on the student’s transcript
