Procedure for Requesting a Workplace Accommodation
An employee communicates his/her request for accommodation to their Academic or Non Academic Administrator by completing the Request for Workplace Accommodation Form. [1, 2] Employees are encouraged to communicate with their union representative, if applicable, regarding their request.
 In limited cases, the Academic or non Academic Administrator may be required, in consultation with the Department of Human Resources or the Office of Faculty Relations, to make enquires of the employee based on observable changes in employee behavior and/or performance.
 In cases in which the employee is absent from work, the University may facilitate a return to work.
