To apply for entry into the Bachelor of Nursing (Post-RN)
Nurse Practitioner Primary Health Care Program please fill out this
form. You must be a registered student for the current or preceding
semester at Memorial University. If you are not currently a
registered student at Memorial University please visit the Web Admissions page for
the Office of the Registrar. Once you have been accepted as
currently registered student, you may then apply for
Admission/Re-Admission to the Bachelor of Nursing (Post-RN)
Program. Please note that 1 Referee must submit a letter of
reference to the School of Nursing as part of the application
process (see Application Reference Form).

One of the requirements for entry in the Bachelor of Nursing
(Post-RN) Nurse Practitioner Primary Health Care Program is the
completion of the Health Assement Form. Please print out this form
and fill in the appropriate information. A portion of this form
will also have to be completed by a Medical Physician. Please note
that both the Health Assessment and the Immunization Requirements
For Nursing forms must be submitted together.

The Immunization Requirements For Nursing Form must be
completed by a Medical Physician or Nurse. All titre tests and
immunzation records MUST be attached when submitting this form.
Please note that both the Health Assessment and the Immunization
Requirements For Nursing forms must be submitted together.
Students applying for admission into the Bachelor of Nursing
(Post-RN) Nurse Practitioner Primary Health Care Program must have
2 letters of reference concerning your aptitude for profesional
education provided by your most recent manager/supervisor. Please
note that applicants must fill out Part A of the Application
Reference form and then forward this form to the referee.
