COVID-19 Self Assessment
To prevent the spread of COVID‐19, anyone accessing campus, offices or conducting field work including faculty, staff and students are required to complete a daily self‐assessment. Please submit to supervisor. The answers to the questions in the assessment will determine if access is permitted. The results are valid for 24 hours. This assessment is not meant to be used as a tool to assess your overall health nor does it replace a visit to your primary care physician. This tool is meant to assess new or worsening of long‐standing symptoms and no other symptoms such those related to seasonal or environmental allergies.
The following instruction will guide you through the assessment tool within the MUN Safe App. The paper self-assessment form is also available for any person that does not have the app. Get more information about MUN Safe and downloading the app.
The COVID-19 Self-Assessment can be found on the home screen of the MUN Safe App. When you select this button you will be taken to the home screen of the assessment.
The assessment asks a series of questions.
- If you answer “No” to all questions (you have identified that you have no symptoms or risk factors) you will get a message that you are approved with a green signal to proceed to campus, office or field activities.
- If you answer “Yes” to one or more questions (you have identified that you have symptoms or risk factors), you will get a message that you are denied access with a red signal and asked not to proceed to campus, office or field activities.
You can review your Assessment results by choosing the “View Assessment Results” button at any time.
Assessment results are valid for 24 hours. You must retake the assessment on any day that you wish to resume university related work on-campus, in-office or field activities.
Please Note: Prior approval must be granted from your unit to return to work based on your unit operational plan. This assessment alone is not adequate approval to return to work or field activities outside of the home.
If you wish to send your assessment result to a supervisor or designate for any reason, you can do so by clicking on the red or green signal (QR Code square). This will open a secure version of your assessment that you can share by email, text, etc.
Authority and Privacy
This Self Assessment Form has been created by Environmental Health and Safety (Office of the Chief Risk Officer), based on recommendations made by the Province of Newfoundland and Labrador and the Government of Canada.
The information on the assessment is collected under the authority of the Access to Information and Protection of Privacy Act, 2015 (SNL2015 Chapter A-1.2) It will be used only for statistical analysis. No identifiable personal information is collected during this assessment.
