* Please Note: All efforts have been made to contact eligible schools. Each eligible school will be contacted via email AND mail. Emails will be sent to the schools Guidance Counsellor. Both the email and the mailed envelope will include the exact same information; one copy of the Application Form and one copy of the Guidance Counsellor/Principal Letter, inclusive.
HORIZONS Application Form Instructions
1. Eligible students are to complete all sections of this Application Form and submit a Resume and Leadership Essay to the Guidance Counsellor or Principal.
2. The Guidance Counsellor/Principal reviews applications and chooses one student to nominate for entry into the Johnson HORIZONS Program. Students are to be evaluated on the basis of the quantity and quality of leadership and teamwork ability as demonstrated in the students' volunteer, school, community and extracurricular involvement.
3. The Guidance Counsellor/Principal will fax, mail or email the chosen student's Application Form, including the student's Resume and Leadership Essay to the address below. Deadline for Application Forms is Friday, May 10th, 2013:

Please Note: No school is guaranteed a Johnson HORIZONS Program recipient and all schools with a grade nine class are eligible to submit a nomination. The successful completion and acceptance of applications does not mean that the nominated student is a scholarship winner. All nominations received by the deadline will be considered by the HORIZONS Selection Committee who will pick 50 students deserving of the scholarships.
