The "Health & Safety Culture at Memorial Survey" is an anonymous survey that offers all Memorial employees an opportunity to contribute to an assessment of Memorial’s health and safety culture. “Health and safety culture” is generally defined as the extent to which organizations and individuals are committed to working in a healthy and safe manner as demonstrated by their attitudes and their actions.

The SafetyNet Centre for Occupational Health and Safety Research at Memorial is carrying out this survey on behalf of Environmental Health & Safety and with support from the Centre for Institutional Analysis and Planning (CIAP).

Participation in the survey is voluntary. The survey should take about 15 minutes to complete. Survey responses cannot be traced back to individual respondents so are fully anonymous.

SafetyNet will present the findings of this project to Environmental Health & Safety in a Final Report available through their website. We may also publish analyses of the results in peer reviewed journals.

Consent:

Completing and submitting the survey implies that participants:
• understand what the study is about and what you will be doing.
• understand that they are free to withdraw participation from the study by closing the browser window or navigating away from this page prior to submitting the completed survey (paper versions of the survey do not have to be submitted).
• will not have to provide a reason for not completing the survey and not completing the survey will not affect you now or in the future.
• understand that responses cannot be removed once the survey has been submitted.

By consenting to this survey, participants do not give up their legal rights and do not release the project team from their professional responsibilities. 

Privacy Statement: Any personal information collected for this project is done so under the authority of the Memorial University Act (RSNL 1990 Chapter M-7) and is used for the purposes of contacting potential participants for focus groups and prize draw winners for the “Health & Safety Culture at Memorial” project. This information will not be saved beyond the project nor connected to any other data collection.
