Mathematics Placement Test Information
PHIA 1000
The Personal Health Information (PHIA) - Health Privacy Law Training 1000 on-line course is mandatory and a requirement of Memorial University. This course has to be completed prior to beginning any clinical placements associated with clinical courses in Newfoundland and Labrador.Students have to register for PHIA 1000 through the Registrar's Office. The Course Reference Number is 78225 for the winter semester. Students should review the Home Page on D2L prior to completing the 45 minute track for health care providers. Following the completion of the course, print the Record of Achievement form and the Oath of Confidentiality form. The Oath of Confidentiality form has to be completed and notarized by a Commissioner of Oaths in the community.The original copy of the Record of Achievement and the Oath of Confidentiality Form has to be submitted to the Clinical Program Administrator, School of Nursing.
