Report an Incident

Any incident that involves injury to a person or damage to property, or had the potential to do so, must be reported to the Environmental Health and Safety within 24 hours of occurrence.

The Memorial University Incident Report Form must be completed for every incident, even if there were no injuries sustained. If the incident involves a student or visitor to campus, the report form must still be completed.

Information for employees/supervisors/managers

If an employee suffers an injury while at work:

1. Report your injury to your supervisor immediately.

2. Seek medical attention or call emergency contact numbers applicable for you area.

3. The supervisor must complete an incident report form and return it to Environmental Health and Safety within 24 hours of the incident. 

Where supervisors are unable to fully complete the form within 24 hours, the form, with as much information as is available at the time, should be submitted with the remaining information to follow later. The supervisor and employee review the recommendations of corrective action with a focus on prevention of recurrence.

Please note:  If it is a serious incident it must be reported to Environmental Health and Safety immediately (864-3786). A serious incident is defined as any of the following:

  • Death
  • Collapse or structural failure of a building, tower, crane hoist, temporary construction support system. or excavation
  • Explosion, fire, or flooding
  • Fracture of a major bone
  • Amputation, or cuts where major blood volume is lost requiring hospitalization or time off work
  • Loss of sight
  • Third degree burns
  • Unconsciousness resulting from concussion, electrical contact, or asphyxiation
  • Poisoning
  • Any injury resulting in paralysis
  • Any other injury likely to or had the portential to endanger life or cause permanent disability.

When an employee is injured at work, the injury should be reported the the Workplace Health and Safety Compensation Commission (WHSCC).

Incidents that do not result in injury but had the potential to (near miss) 

1. Report the incident to your supervisor immediately.

2. The supervisor must complete an Incident Report form and return it to the Environmental Health and Safety within 24 hours of the incident.

Where supervisors are unable to fully complete the form within 24 hours, the form, with as much information as is available at the time, should be submitted with the remaining information to follow later. The supervisor & employee review the recommendations of corrective action with a focus on prevention of recurrence.

Responsibilities:

Supervisors/managers

1. Fill out Incident Report with employee / student if possible.

2.  Submit the Incident Report to Environmental Health and Safety within 24 hours of the incident occuring.

4. Cooperate during the investigation. 

3. Implement effective and practical action to prevent a recurrence of the hazardous condition. May require short-term corrective action measures to control immediate hazard, and long-term measures to effect change.

4. Complete the supervisors/managers portion of any WHSCC forms in the event of an injury. 

5. Provide employees with WHSCC form 6 in the event of injury.
