Group Travel
Memorial University has a diverse environment involving groups, clubs and societies, and these are an integral part of campus life.
These groups are sometimes required to travel for events. For these occasions ERM has compiled resources and forms to make the occasion a safe and successful one.
Forms:
- Acknowledgement of Group Travel Policies and Procedures
- Group Checklist
- Group Travel Detail Form
- Request to Drive on University Business
- Group Driver Agreement
- Personal Vehicle Use Agreement
- Health Statement and Medical Release (only required for travel outside of Newfoundland)
These forms must be completed by the Coordinator in charge of the Group and/or the Group travel. The Coordinator will then inform Enterprise Risk Management that the relevant information has been completed and if necessary forward any completed forms to ERM.
