How do I complete the copyright box on the Printing Services Requisition Form?
The purpose of the copyright box is to indicate both the copyright status of the work that you wish to have copied and whether the appropriate permissions have been received from the copyright owner. The form must be signed by the appropriate person and at least one circle checked or Printing Services will not be able to print your document(s).
If the work being copied is all your own creation and you are the owner of the copyright, check circle 3.
If the work is not your own creation, or is primarily your own but contains excerpts from another source(s), you must declare under what terms the material from the other source(s) is being copied. Check circles 1, 2, 4 or 5 (or choose 3 and 1, 2, 4 and/or 5) as appropriate.
If the work being copied includes material in the public domain, check circle 4.
If the examination is entirely the original creation of the instructor(s), check circle 3.
If the examination contains material for which the instructor is not the copyright owner, but which may be copied pursuant to an educational exemption, check circle 1.
If the examination contains questions from a licensed test bank, check circle 4 and attach the appropriate permissions to the Form. These permissions often form part of the documentation that accompanies the test bank materials.
If the examination contains a business case or any other content that is normally available on a commercial basis, check circle 4 or 5 as appropriate and attach the related permissions to the Form. If you require assistance securing permissions please contact the Copyright Office. Send these examinations to the Copyright Office with the completed Printing Services Requisition Form attached. The Copyright Office will forward the package to Printing Services.
The person who signs the copyright box on the Printing Services Requisition Form should understand the content of the manual and the copyright status of that content. If the course manual is a collaborative work, who should sign the printing requisition must be agreed upon by the authors of the work or by the department. The manual may include materials with different statuses; in which case, more than one circle may apply. Check as many circles as necessary, supplying the appropriate documentation for each.
Send course manuals with the completed Printing Requisition Form plus any relevant permissions to the Copyright Office. The Copyright Office will forward the package to Printing Services.
