Service Request Forms

• Device Authorization Tool (DAT) - submission of this form is required to gain access to Memorial University's Wireless Network. You will need a valid MUN Login account to complete this process.

• Device Authorization Tool (DAT) For a Sponsored Guest - this form allows an authorized university staff or faculty member to create a temporary Wireless Network account for a campus visitor. You will need a valid MUN Login account to complete this process.

• Network Service Request Form - submission of this form is required to connect to Memorial's Wired Network.
  
  
• On-line Help Request Form - submission of this form allows MUN students, faculty and staff who require assistance on a computing related problem to submit an electronic request.
  
  
• Distribution List Request - submission of this form is required to create a Distribution List.
  
  
• McAfee Endpoint Encryption Account Access - this form must be completed when a laptop encryption request is submitted, it defines which users have access to a particular laptop.

• VPN Application Form - submission of this form is required to request VPN installation on a University-owned laptop.

• SLA Schedule E and F Forms - for voyager access and exchange services.
